Dementia Advisor Service for Stoke on Trent
Launched in September 2010 the new service provides two dedicated full time Dementia Advisors along with two support workers. The team will work with individuals who have been newly diagnosed with dementia to offer support and advice, helping individuals better understand their condition and access support within their local communities.
The dementia advice and support team will provide practical advice, guidance and emotional support to individuals and carers, providing comfort and reassurance that they are not the only people in such circumstances. This gives a much-needed insight into how best to cope with the changes in their lives by improving their well-being. There is a focus on supporting older people and their carers who are at danger of becoming, or are already, socially isolated and excluded from their community, and who need practical support and advice to remain in their home environment. This will support older people with early stages of dementia to remain in their community with access to a wide range of support.
The service covers the whole of Stoke-on-Trent, including those patients who may life outside the City but still have an NHS Stoke-on-Trent GP. Approach is delighted to welcome its two dementia advisors, Gary and Emma who have been joined by Janice and Kelly, the two new support workers.
Referrals to the team can be made through memory clinics, social workers or directly by individuals. |
