What we do?
Our Dementia Navigator service at Approach will provide support and advice to older people with dementia and their carers.

How does it work?
Our Dementia Navigator will visit individuals and their carers in their own homes (or wherever they feel most at ease) and help them to access local services which are appropriate to their own needs.

This service is free and confidential and gives quality information and guidance to newly diagnosed dementia sufferers and their carers.

This service has been made possible with funding from Supporting People, Staffordshire and will be delivered in partnership with Social Care and Health.
